Onyx Technology, LLC (“Onyx”), a vendor of Independent Care Health Plan (“iCare”). Onyx experienced an event that may involve personal information. iCare has contracted with Onyx to help patients get access to their electronic health information when they need it. We do not think that information has been misused because of this event. We are giving individuals things they can do to protect their information.

What Happened?

On June 28, 2022, Onyx learned of an attack on their computer systems. Onyx immediately reviewed their systems and hired a security firm to investigate. Onyx regained access to its systems on July 7, 2022. A server may have been removed or accessed beginning on March 29, 2022, and ending on June 28, 2022. On July 15, 2022, the security firm found that some information related to individuals may have been accessed.

What Information Was Involved?

The personal information that was at risk includes:

  • Name
  • Date of birth
  • Address
  • Phone number
  • iCare member ID Number
  • Medicare ID Number
  • Date of service
  • Provider’s name

We Know That Individuals May Be Worried About What Took Place

We would like to help individuals safeguard their information from potential misuse. We have partnered with Experian Consumer Services. Impacted individuals are entitled to free credit monitoring for two years.

What Individuals Can Do

We are not aware of any actual misuse of information. We encourage individuals to remain vigilant against incidents of identity theft and fraud by reviewing their account statements, Explanation of Benefit forms and monitoring their no cost credit reports for suspicious activity. We are encouraging everyone to review the document titled Steps You Can Take to Help Protect Your Information, that was enclosed with the notification letter. It contains information on what individuals can do to safeguard against possible misuse of their information.
